Flame failure. The burner lit and the flame signal was then lost. Rinnai's causes are gas supply first — a closed valve, an empty tank, inlet pressure sagging when other appliances fire — then the flame rod itself: carbon or a white deposit on the rod, a cracked silicone sleeve, a loose electrode, then grounding and the gas valve.
